Conformance & Remediation Form
You will need to send this form to your Textbook & Content Publishers. Your publisher will fill out this form and email it back to you. You will need to forward a copy of the Conformance & Remediation Form to Distance Education.
Alternate Access Plan
You can use this form if making the content accessible will hinder the learning outcomes of other students. For example, if providing alternative text to an image would also provide the correct answer, you would instead use an alternate access plan. The alternate access plan would detail how you would make the content accessible in the event that a student needs an accommodation.
